Who is responsible for the processing of your personal data?

The person responsible for the treatment is Fundación Instituto Valenciano de Oncología, with registered office for this purpose at Calle Profesor Beltrán Báguena 8, CP 46009 Valencia (Spain).

The IVO has formally appointed a Personal Data Protection Officer, and also has the following email enabled: dpo@fivo.org

For what purposes do we process your personal data?

The IVO, through the website, collects personal data for the following purposes or uses:

  • Contact form: Personal data of a merely identifying nature (Name and / or surname) and contact (email, telephone) will be processed to meet the query, opinion, suggestion or other type of communication that you direct to us, in relation to the activity carried out by the IVO.
  • Form ‘I want to be a patient’: Personal data of a purely identifying nature (Name and / or surname) and contact (email, telephone) will be processed to initiate the first contact from the IVO with you; for more information about the oncology health care you need.

What is the legitimacy of the IVO for the processing of your data?

In accordance with the aforementioned purposes or uses, the processing of personal data is legitimized based on the following conditions of legality:

  • In relation to the data obtained in the ‘Contact’ form, the legal basis of legality or legitimacy is the consent that you express by completing and sending the data, freely and voluntarily.
  • In relation to the data obtained in the ‘I want to be patient’ form, the legal basis of legality or legitimacy is the need to manage the pre-contractual relationship with the IVO.

How long do we keep your data?

The data will be kept for the time necessary to comply with the purpose for which they were collected and to determine the possible responsibilities that may arise from said purpose and the processing of the data.

To which recipients will your personal data be communicated?

The personal data processed by the IVO, through the aforementioned forms provided on the website, will not be transferred or communicated to third parties, except in the cases provided, according to Law.

 

What are your rights in relation to our processing of your data?

The IVO informs that it has the right to access your personal data and obtain confirmation on how such data is being processed. Likewise, you have the right to request the rectification of inaccurate data or, where appropriate, request its deletion when, among other reasons, the data is no longer necessary for the purposes that have been collected by the IVO.

In certain circumstances, you may request the limitation of the processing of your data, in which case the IVO will only keep them for the exercise or defense of possible claims.

Please note that deletion or limitation of processing is only possible if and to the extent that such processing is based on consent or legitimate interest. If it is based on consent, you have the right to withdraw it at any time, but the withdrawal of the same does not affect the legality of the treatment based on your consent before its withdrawal. In the event of a deletion request, we may retain a copy of your personal data for our record-keeping purposes and to avoid accessing your personal data in our systems following your request.

Likewise, also in certain circumstances you may object to the processing of your personal data for the purpose informed by the IVO. In that case, the IVO will cease processing personal data, unless there are legitimate reasons, or to guarantee the exercise or defense of possible claims.

Finally, you can request the right to portability and obtain for yourself or for another service provider certain information derived from the contractual relationship formalized with the IVO.

You may exercise these rights by writing to the postal address indicated above or to the email dpo@fivo.org.

It is necessary to prove the identity of the person who exercises their rights, by sending a copy of their ID, NIE, Passport or equivalent document.

The IVO will provide you with the requested information within a maximum period of one month from receipt of the request. That period may be extended by a further two months if necessary, taking into account the complexity and number of applications.

You may withdraw consent at any time, in the event that consent has been granted for a specific purpose, without affecting the legality of the treatment based on the consent prior to its withdrawal.

You may file a claim with the Spanish Agency for Data Protection (www.aepd.es). However, in the first instance, you can file a complaint with our Data Protection Officer (dpo@fivo.org).
